We had 3 fatal accidents in 3 years at Hahn Air Base, all of them CFIT Accidents, Controlled Flight Into Terrain, it happens, and nearly happens to all fighter pilots, the Commander of the Air Forces in Desert Storm almost got killed on a range in his early flying career. Not everybody is a Chuck Yeager.

Then again, the hardest point I ever ran into was the speedbrakes. Sucks when you're washing the jet and you're cleaning the ventrals and someone opens up a brake and doesn't warn you - you turn around and BAM slam right into the thing. Hurts real good.

More intense training for Iraqi invasion .
Also , This aircraft has a history of getting pilots into blackouts because of intense High G-forces . A third possibility : The f-16 is a fly by wire plane ( the F-117 stealth fighter black jet is also fly by wire , and made from F-16 parts ), which means it's airlerons and other flight control systems work mostly if not all electrically , not hydrolically .
Thirdly , this plane has been thought to have small pieces of metal or screws get into the wiring and cause crashes .
