This was very difficult airspace - the approach end of RW33 is only 2,000 feet from Route 4 at the closest point, due east. That’s WAY too close for safety at this very low altitude with fixed-wing traffic on short final....
Flying this route on NVG’s was beyond this crew’s ability, at least on that night.
what are the best & worst scenarios in your opinion
How does one get to be a Captain in the 12th Aviation Battalion, and only have 500 hours of flight experience?

I agree with Col. Joe’s assessment. The obvious immediate action is
1) to S**TCAN the unit commander.
2) Then order a stand-down of the entire US Army helicopter force for 2 days for a safety review with emphasis on airspace.
3) Revise all airspace near major airports to eliminate the possbility of VFR traffic intersecting the flight path of IFR traffic close to landing.

Withholding the name only allows the conspiracy nuts time to get revved up. Mission accomplished.
Thank you for posting the summary.
“As I posted a couple hours ago, the name would be known by Monday. Ahead of schedule, I figured it would be leaked.”
Is this the rumored trans pilot? I think the name being bandied about is Jo Ellis. With Trump’s DEI policies and if this is the case, someone is in deep deep doodoo. The other person who hasn’t been mentioned is the ATC who left the shift early. I would like to know who that person is.
PS.
ATC failed to request PAT25 heading and then . . .
ATC failed to tell PAT25: “Check CRJ at your 11 o’clock [bearing 150 deg compass heading] over land.”
ATC failed to tell PAT25: “Check CRJ at your 10 o’clock [bearing 120 deg compass heading] over land.”
Some helo pilot online said a helo flying over a black body of water at night ca:n rise a hundred feet in mere Seconds and go initially unnoticed. He said a BlackHawk can do it even faster. I’d guess this flight got ahead of the pilot and the check pilot at the wrong time and they never saw the jet or their altitude.
“what are the best & worst scenarios in your opinion”
Best scenario is either the pilot or the ATC who left their shift early are DEI hires. Which would prove Trump right and would probably cause cities to burn. Hopefully in cities like Chicago, Portland where there are sanctuary policies with bad actors and gender dysphoric’s running around.
Worst scenario? Pilot left a suicide note and the media goes after Trump for not helping the mentally disabled.
Most probably scenario? ATC doing double duty and missed the pilots call. Pilot took eye off the gauges and kept going up. Perfect storm especially in a very busy and dangerous environment. It’s called a training accident.
As I understand it, the CWO was an instructor and the Capt was the Pilot Under Evaluation. While the Capt was the “Pilot in Command” of the aircraft when she was flying it, the CWO would have taken over in case of emergency, or if the Pilot Under Evaluation failed to operate the aircraft safely.
It’s not completely clear who was the Pilot in Command when the collision occurred. The CWO was communicating with the ATC, and he was the one who requested “Visual Separation”, essentially saying “I see the CRJ and will avoid it, request permission to continue”. It’s possible he had taken control of the aircraft. If the Black Hawk didn’t have a Cockpit Voice Recorder, we may never know.

Job duties?
Oh?
Has that been verified?
Warrant and Commissioned Officers can both fly in Army Aviation. In our ROTC cohort, we had two cadets who branched Aviation based on their FAST test scores, application packet, etc.
CBS NEWS:
“Lobach also served as a White House social aide during the Biden administration, Brown said. Just last month, she escorted Ralph Lauren through the White House when he was among those awarded the Presidential Medal of Freedom by former President Joe Biden.”
Other networks say she was an Aide to Joe Biden.
