If masks are so full of magic how come prior to 2020 they are not used??

I have been hospitalized several times in the last few years before COVID and the only time my care givers were masked was when I was admitted for a nasty pneumonia of unknown origin. For the first few days of my stay everyone entering my room was masked, had gowns and gloves.
