To be clear: I wasn’t saying that ATC had made an error (although their ambiguous wording was a contributing factor, leading to several communication changes). I was saying that even *if* ATC *had* given a clear go-ahead, it seems the captain wasn’t using due caution in proceeding.

After having researched a little, the issue isn’t Jet-A, which certainly predated Tenerife, but AMK additives added to Jet-A. However, from what I gather the show I had seen contained a LOT of hype.

Analyses of aircraft accidents led to the concept of the error chain, in which breaking only a single human error linkage will avoid a major event. If small errors in safely completing each increment of a task can be avoided, the cumulative result will be avoidance of a major incident leading to loss of life or property.
