The Foley Catheter does require a water bag that must be worn higher than the catheter.
The water from the bag keeps a 'balloon' inflated and the inflated balloon keeps the catheter in place and prevents leaking.
And she is definitely wearing a catheter as evidenced by the photo showing a catheter tube snaking from her crotch down her leg.
There is a built in valve that keeps the Baloon inflated.
