These polls you're citing are using old data that goes back into August.
Trump leads Clinton in at least 2 newer Ohio polls, as well as in FL and NC, and is very close in MI, PA, and VA.

Trump is ahead in all polls from data collected after 9/1/16

The ABC/Washington Post poll has Hillary up by 5 points, but that's well within the MOE. I tried to find the party splits, but couldn't. I suspect they would be quite informative.
Given the trends in party affiliation that have occurred in the last few years, any polls that rely on, say, 2012 party affiliation data should be highly suspect.
To reiterate, using only SEPTEMBER polling data:
OHIO: Quinnipiac has Trump +4 over Clinton in a 4-way race, and +1 in a 2-way race.
NC: Suffolk has Trump +3 over Clinton in a 3-way race.
Trump is also up over Clinton in several September national polls.

ABC Wash poll Partisan divisions are 34-24-33 percent, Democrats-Republicans-independents, in the full sample, and 36-28-31 among likely voters.
To: jennychase
ABC Wash, they are over sampling Dems by 10 and 8. If you see enthusiasm level of GOP is way high.
It means Trump is leading here also.

Factoring in the Bradley Effect and the normal thumb on the scale for the Wicked Witch, Trump is likely ahead by at least 7%.
