They actually wear a “normal” pair of underwear. The boxers that you see hanging out are actually an accoutrement to the whole “ensemble.”
I’ve spoken to some of the “men” at the company next door to us, and they explained this to me as simply a social fashion statement. Perhaps I’m white and won’t/don’t understand, but I agree, this is one of the more ridiculous fads/trends along the lines of platform shoes.

My daughter once brought home a guy that had the droopy drawers. I took him out in the garage and showed him the pneumatic staple gun with 2" staples, and told him that if he ever showed up like that again, I'd keep his pants up with staples. He just never showed back up at all.

Maybe for some it is..I have seen some that leave nothing to the imagination and its clear that is the only pair they have on and the cloth is so thin you can see more detail of their backside than I care to
