Basically, the medical protocol is to use non-opioids first, then escalate to non-opioids with combination drugs that help the pain relief.

Finally, a patient is put opioids [such as morphine].

However, patients can build up a tolerance to such drugs.

It basically gets down to: the medication is not working, and you would have to kill the person outright. This is none as murder.
