Who do you think selected those who are in charge of the party? There are local elections. The folks who win those elections select the state leaders, etc. They did not just magically get to be leaders in the party.
Who writes the rules?
One of the characteristics of a well-regulated Republic is that the rules are far more persistent than the principals. While it may be that some rules happen to benefit those in power at the time they're written, most of the rules will have been written by earlier people with no attachment to the current office holders.
It means little for people at the bottom to have a voice, if the people at the top have exclusive control over how that voice is interpreted.
