I'm not overly-qualified to discuss this, because when my children were young we really could not afford to go out much, anyway. So trips to a restaurant were special occasions and the kids acted well.
That said, this all sounds like second-hand smoke. Not everybody likes it, and the private business owner has to make a business decision about what the clientele should look like.
And, as I recall, parenting was never a part-time job. It started with "I'm pregnant", and continues to this day when the adults we still see as children are in their 20's.

I don't think it's reasonable to drag a child to a grown up place when she's sick
I dont think it is reasonable to drag a child to a grown up place when shes sick or drag her anywhere but a doctors office.

Ive noticed over the years that rowdy kids will often show more respect for strangers than for their parents. They know their parents wont do anything but maybe talk but they are unsure of the stranger. Often it only takes a stern look directed at the kid.
