Could be the vitamin D from the sun or the possibility that many were killed by others or from ODs, alcohol abuse, infections and other self inflicted health problems years ago.
Darwin thins out that herd on a daily basis.

Population density, poor diet, poor health and unsanitary living conditions tracks poverty closely. So how do they know it's just poverty?

Exactly. Doesn’t matter what the economic system is, having more wealth buys you more of everything you want to buy, including health.
But only a market system gives people an incentive to take risks for the health of those in the future.
