Why am I not surprised. The second document was proof that as of May 20th the bill had not been paid. Supposedly it had been paid on May 10th per DUmmy Andy. Now we have a bill from May 20th asking for payment. Losers all.

Yeah, but just for the sake of historical integrity and accuracy, if someone sees an image or document posted, it's a good idea to save it. Better to save too much, than to save not enough.

My guess is the reason the second doc was removed is that after reading the info here, they realized that they screwed up with the dates and the amount owed.
Will they never learn....can't really ever take something back that has been put out there on the internet.

That is too funny. Just looked at the site. We could all fill out the exact same form and post it. The form is meaningless to his case.
Only one thing I find amazing. By using the doctors business card for the info section, it deleted the spot on the form for the person's signature who filled out the form!
This whole form is bogus! It is worthless as any kind of proof. No signature from who filled out the form means no one would except it as any kind of application. If the doctor doesn't sign it why would you even submit it?
Looks to me like they pulled it off of the net sent it through their own fax/copy machine in order to put bogus dates, fax numbers, zip code, the whole bit. This is easily done in the set up portion of your machine. You also get the added look as if it has been through several machines to make it look authentic. There is no way the fax machine would "white out" the top and cut the words "What Kind of Financial" off the top. It would merely print over the top so it would look garbled.

I still check to make sure I'm posting a reply right. I don't think my brain could handle how to save anything or screen capture. Fortunately, my brain is very good at saving and screen capturing all by itself.
