I believe hillary has cirrhosis of the liver from her alcoholism. Her abdomen seems to change is distension, as if she has been having to have paracentesis (pulling fluid off of the abdominal cavity). Her cough is consistent with pleural effusion causing the persistent cough. I don't know for sure, but it is consistent with her constellation of signs and symptoms.

I do not know what to make of the 'indention' in her tongue.
