A government watchdog agency said an estimated $106 billion in payments were made in error last year: meaning they were the wrong amount, went to the wrong person or lacked sufficient documentation.
These payments include tax refunds, unemployment benefits, Social Security benefits, and Medicare and Medicaid coverage.
The improper payments could be even higher since not all agencies reported to the Government Accountability Office.
The Department of Agriculture's School Breakfast program, which provides free or reduced-price meals to needy students -- had the highest error rate in 2013, at 25%.

School breakfast for the needy, is false.
I live in a great neighborhood with some of the best public schools.
The public schools offer free breakfast to kids who in no way, shape, or form need it.
The kids might go in for a juice, but have to get a tray with everything. A mom told me that most kids throw almost everything away.
It’s a,huge waste.
And the school breakfast program is terrible for children,a high carb disaster guaranteed to promote obesity.
As a cafeteria monitor, I have seen sweetened cereal, a slab of butter-soaked Texas toast, and chocolate milk being served. Somebody is making money serving slop fit only for fattening pigs for slaughter...but mom can’t be bothered to get up and scramble an egg for her child.
They can’t even be bothered to pour a bowl of cereal and put out a piece of fruit.
Why should they, gov’t does it for them, they don’t care if their kids eat right or not.
I am from the Brown Bag generation, we ate at our desk K-8. We bought a 5 cent half pint of milk. Then we went out side for the rest of the 45 mins. AM/PM 15 min recess time. Weather factor sent you to the gym to run off excess energy, we did not have alphabet soup issues they are drugging boys for back then either.
Another way of circumventing congress and the redistribution of our tax dollars. Would like to see a list of people and areas the money went out to.
Many municipalities now have summer lunch programs. Like mine. I live in a part of the country where procuring food is not difficult, and there is no lack of chubby young’uns running around. Or I wish they were running around instead of playing with their stupid x-boxes or whatever. Whatever happened to families feeding their children?

Even more outrageous is that IBM offered to remove blatant causes of fraudulent payments for FREE. That’s at least 30 billion per year right there.
Removing the fraudulent payments listed here would be relatively easy to add to the processing.
