No idea - it could have been an accident, incompetence (separate from accident), sabotage or a deliberate attack. No way to tell, but if it was hostile action, I wouldn’t put it past an attacker to use even a genuine hospital as a distraction. Remember, Israel and the Western powers aren’t the only enemies Iran has and a lot of them have less scruples than even Iran.
Or it could just be someone bought a bad set of gas handling equipment and installed it everywhere.

Iran's foreign policy centering around destruction of the little and great satan, I think this is a good accident. Far better than the Obama policy, sending Iran cash.
