I believe it is a real photo and the photographers are taking pictures of something beyond Melania. Not a big deal really. Will be good for Memes for all sides.

Looking at the furnishings, the flowers on the coffee table and where the cameras and mics are pointed, it seems that Melania was not alone but at that presser, sitting across from President Trump & Japanese PM Abe and she was looking at and listening to one of them talking.
