Is the steel from China?That was my first thought. This was a few years back, but I knew someone at a large construction company who bought steel beams from China. They soon learned that they had to oversee the testing process on site because they had been receiving good test results and a lot of substandard product.

No it was US steel, but third party special inspection was pushed aside by the contractors and welders unions in favor of their own CWI training certs as sufficient, third party quality control Ultrasonic Testing is a thing of the past
but most likely cause is a wacky grandiose architectural design that sacrifices structural integrity
