The “The Siege”, about the attack on the Mumbai hotel, is descriptive of how these attacks go down. They are planned to have many diversionary attacks throughout the city, to confuse the police as to what the focus of the attack is and where to send response forces.
If one part of the attack does not come off, then those attackers who remain are to improvise and do the best they can.
These kinds of attacks are meticulously planned and carried out.
