The requirement that insurance companies cover people with pre-existing conditions is inextricably linked with the mandate (that was the point of Cooper's question).
The mandate for everyone to have insurance is the mandate which says that everyone must purchase insurance. That mandate is what made it affordable for the insurance companies to offer coverage to people with pre-existing conditions. Basically you have to force all of the healthy people to buy insurance so that the insurance companies can afford to offer insurance to those who already have serious health problems.
