You don't get 2m people simultaneously visiting at the Vatican. The stampede is the result of inadequate crowd control combined with an inherently difficult situation. NY's Times Square area has 1m people every New Year's Eve and no stampedes despite general inebriation because of 10,000 cops, crowd control barricades everywhere and crowd barriers called skyscrapers, whose glass doors could have been shattered to alleviate any crush. Whereas the Saudis had 2m people to deal with, but all out in the open.

2m people out in the open presents a very difficult crowd control problem. Scale up the Who concert numbers 100 times, and you have something in the ballpark of the Hajj casualties. Most of the deaths are from asphyxiation, not from being stepped on.
The Who were in the midst of the United States portion of their 1979 world tour and had already played Passaic, New Jersey, New York City, Detroit, and Pittsburgh before arriving in Cincinnati. The concert was a sellout, with 18,348 tickets sold. The majority of these, 14,770, were unassigned general admission tickets that were first-come, first-served.

The Victoria Hall disaster, in which 183 children died, occurred in Sunderland, England on 16 June 1883 at the Victoria Hall, which was a large concert hall on Toward Road facing onto Mowbray Park.
On 16 June 1883 a children’s variety show was presented by travelling entertainers Mr and Mrs Fay.[1][2]
At the end of the show an announcement was made that children with certain numbered tickets would be presented with a prize upon exit. At the same time entertainers began distributing gifts from the stage to the children in the stalls. Worried about missing out on the treats, many of the estimated 1,100 children in the gallery stampeded toward the staircase leading downstairs.[3] At the bottom of the staircase, the door had been opened inward and bolted in such a way as to leave a gap only wide enough for one child to pass at a time. It is believed this was to ensure orderly checking of tickets.[4] With few accompanying adults to maintain order, the children surged down the stairs toward the door. Those at the front became trapped, and were crushed to death by the weight of the crowd behind them.[2]

When information first came out about the San Bernardino killer, it was mentioned that he had travelled to Saudi Arabia. But it was dismissed as no big deal, because he was merely going to the Hajj.
That’s a huge deal. Muslims are supposed to do this at least once...before they die. Of course he went to the Hajj before the onset of sudden jihad syndrome.

I would like to the Hajj celebrated on a weekly basis.
This is just a guess, but the Saudi hire people from India to do manual labor, be maids, etc. Wealthy Saudis may bring them along to keep the tent tidy for the Hajj.
Pagans enjoy a little human sacrifice to their idols. Trampling may not have the flair of cutting out hearts on an altar, but what it lacks in presentation, it makes up for in volume.
