Once you establish a formal plan to respond to a crisis like this, and hire public safety personnel to do the job, you can be sure that the job will never get done effectively. The problem is that the plan and the personnel -- by definition -- get bogged down in various rules, regulations and oversight protocols that make it impossible for them to do the job effectively.
That's why the most effective people in a crisis are usually the ones who find themselves in the situation on their own (as opposed to as a "first responder"), who put their own skills and wits to work under duress, and who don't answer to a chain of command.

If I were leading that team I would've had a "radio malfunction" right before the stand down order was given. It's sometimes easier to ask for forgiveness than permission, and success forgives many sins.

Wrong. I believe these are federal jobs or at least DC municipal. Someone maybe be laterally transferred, promoted or given paid leave of absence but that's it.
Unless, of course, it's like Benghazi where the good guys were hung out to dry and died and the other good guys that tried to tell the truth are intimidated, threatened and punished.

No. Wrong. No one is in deep dodo: these are government workers. They can’t be fired, they can’t be sued, they can’t have their benefits/pensions taken away. They’ll retain their clearances.
Reprimand, perhaps, followed by transfer to cushy job in some other department, or maybe disability for PTSD.
