Experimental drug that alters gut microbiome shows promise for long COVID relief (Simple probiotic / prebiotic combo)

A randomized controlled trial finds that the synbiotic drug SIM01 relieves multiple symptoms of long COVID, or post-acute COVID-19 syndrome (PACS).

SIM01 contains strains of anaerobic Bifidobacterium bacteria (which are probiotics) and soluble fibers (prebiotics) to alter the gut microbiome and possibly modify immune response.

From June 2021 to August 2022, researchers from randomly assigned 463 adult long COVID patients at a single hospital in a 1:1 ratio to receive SIM01 or a vitamin C placebo by mouth twice daily for six months. The median interval between infection and random assignment was four months.

"Decreased abundance of short-chain fatty acid-producing bacteria in the gut of patients with COVID-19 might represent one of the crucial mechanisms contributing to the gut–lung interaction and thereby disease severity in COVID-19," the researchers wrote.

Nearly one third of patients (31%) had been hospitalized during their infections. At six months, ncreases in relative benefit after SIM01 therapy were 47% for fatigue, 56% for memory loss, 62% for difficulty concentrating, 30% for gastrointestinal upset, and 31% for general unwellness.

Receipt of SIM01, infection with omicron subvariants, vaccination before COVID-19 infection, and mild COVID-19 infection were predictors of symptom improvement.

Fecal metagenomic analyses showed that the gut microbiome was more diverse, including more short-chain acid-producing bacteria and fewer genes related to antibiotic resistance, at six months than at baseline in SIM01 recipients.

Correlation of microbial changes with symptoms showed that relief of specific symptoms was tied to distinct compositional and functional changes in the microbiome.

"For instance, Bifidobacterium adolescentis showed positive correlations with alleviation in fatigue, gastrointestinal upset, and memory loss," the researchers wrote. "We also found that alleviation in fatigue and general unwellness correlated with an elevated relative abundance of Bifidobacterium bifidum, whereas alleviation in difficulty in concentration correlated with a positive shift in Bifidobacterium longum."

Despite the blood and fecal improvements, participants getting the probiotic and prebiotic combination didn’t report a much better “quality of life” score.

My guess is that being tied down with everything that remained kept them from being fully whole, lowering their appreciation of what good had come to them.

There were no differences in adverse events between placebo and treated groups.

The supplement had “20 billion colony-forming units of three bacterial strains, B adolescentis, Bifidobacterium bifidum, and Bifidobacterium longum with three prebiotic compounds including galacto-oligosaccharides, xylo-oligosaccharides, and resistant dextrin.”

You can easily fond the bifidum and longum and the prebiotic items can be found in some foods and supplements.

It’s curious, to me, that a good many of the COVID crap diseases have very inexpensive, readily available therapeutics that have been restricted from prescription...

So if you eat yogurt or sauerkraut regularly anyway, you’re in good shape and might not get covid in the first place.

Delayed release is the key because the bacteria will be killed by stomach acid before it reaches the small intestine where it is needed.

The day I started taking this was the last day I could barely think or walk ten feet without stopping to rest.

From then on I have gotten stronger every single day.

After I had COVID I started having severe bloating that was debilitating. Some days I could barely function. I tried a number of things, including probiotics, but nothing worked. Eventually I stumbled across references to Align, a delayed release probiotic recommended by gastroentologists. I tried it and it worked. I have not been bloated since. The one I recommended earlier has ten different organisms instead of just one. It’s the one I use now.

Sauerkraut and Kefir on an almost daily basis. I feel just fine. I also take a multivitamin, plus 5,000-10,000 IU daily of extra D3, K2 (to keep the D3 from calcifying my blood vessels and other soft tissues) and zinc.
