Has anyone submitted to their employer a religious exemption declaration/request against a mandated COVID vaccination policy? How did you format it, and what info did you include?
My company is requiring any exemption documentation be submitted no later than Sept 13th.
Has anyone submitted one and been questioned?
I will add- has anyone submitted a medical exemption? And how did it go?

Since the government is fully invested in this nationwide experiment, don't be shocked if the attempted exemption is rejected.
Just be prepared to have a back up plan if it isn't "accepted."
I have not, but I intend to use Rev 13:16-17 and Rev 14:9-11 as my basis. There was a case not long ago where the verses were shot down because whatever the company was doing didn’t go on/in the right hand or forehead, but now that they are saying we will be limited in our ability to earn money to purchase goods, not allowed in places unless we show our “mark” in the form of a vax card, I think there would be a better than average shot at getting a waiver. If not, I walk...
my wife is going though the same thing right now. we are resisting of course:
the first step is to stall them, been doing that for 2 months now since the county put the pressure on and they complied.
she was already refused a medical exemption by a company doctor (of course). we may consult a separate doctor for her next.
there are various sites out there with justifications written out, medical and religious. we’re cobbling together our own form from them (i think noqreport is one of them).
she’ll put that in at the last minute and see what they do with that.
after that. we’ll have no choice but to refuse it. then we’ll see how they react to that. our point of view is that your good health isn’t worth any job, but as for others...that’s their choice.
Include your Christian Science membership like the other nutsoes.

Maybe the legal profession has been contacted like the doctors and pharmacists, They got letters from the government ordering them not to prescribe HCQ for Wuhan.
For employers covered under Title VII of the Civil Rights Act of 1964, “once an employer receives notice that an employee’s sincerely held religious belief, practice, or observance prevents him from taking the influenza vaccine, the employer must provide a reasonable accommodation unless it would pose an undue hardship.”

Generally, ADA-covered employers should consider simply encouraging employees to get the influenza vaccine rather than requiring them to take it.

Undue hardship to the employer, not you. In the case of the pandemic, hardship isn’t just about accommodation but also the perceived risk of having you around.
And the employer can ask for supporting documentation. Like a letter signed by your Pastor, Priest, Reverend and if they are not on board? You are going to be out of luck and at the Red Sea Moment.
In Hebrew culture, the hand extends clear up to the shoulder. Think about that.

I read that those test increase the risk of meningitis.
I’m not even aware of a major religion that would assist with this.
AFAIK, the Catholic Church opposes exemptions that would help you dodge the jab.
And I think the Church of Christ Scientist also will not push back on vaccines. They go along with the government.
I could be wrong. But I would expect little support and a lot of push back.
I think you can get it from HR. Request for it.
