In a post nuclear war world, most people would die due to lack of prescription medications, and I guess synthetic steroids wouldn’t be all that available either.
You have people mixed in every state though, there would have to be some kind of great separation such as people mass exodus out of certain states.

True, there will have to be a good bit of internal migration, but there’s already a general geographic separation. It’s the South and the Middle, and then there’s the coasts (plus Chicago). Either things start to separate, or there is going to be violence. It’s a shame that it’s gotten to this point, but instead of getting better, it seems to be getting worse.
