AR-15 rifles are not immune to the dreaded kaboom.
Like any firearm, it can fail if improperly loaded with bad ammunition, or maybe other factors.
Does someone smell a squib bullet with another bullet fired into the back of it?
Well, it appears all the parts are there, how come it doesnt fire?
I think, the bottom of an AR-15 rifle is not supposed to look like that. See the rest of the 9 AR-15 fail here

Machines operating under stress sometimes break.
Wife’s car is in the shop, trying to get her to understand this - and that it’s not some conspiracy.
Some people fail to understand that firearms are fairly simple machines that are built to certain specifications.
That they all have a life expectancy determined by the manufacture.
That can fail when those specifications and standards are exceeded.

And that statistically some will fail under normal use.
Make 100,000 having 99.999% reliability and 1 will have a photogenic kaboom.
If I reload and add just a little bit more powder, I can get just a little bit more velocity and stuff.
If I don’t get my reloads’ headspace and OAL correct, bad things will happen.
I learned alot working up insane handloads for a 6’ 586 in my younger days. Destroyed that gun; needed a total rebuild.
20+ yrs later, I’m the poster child for standard pressures and COM hits.
I guess we can fall back on our straight pull Canadian Ross bolt action rifles.
Not long ago I was reading about the matchlock gunners in the 14th and 15th centuries. At one siege 19 gunners were killed when their firearms blew up in their hands.
