If you’re in Medicare or Medicaid it’s apparently illegal to pay cash for services. If they catch you they can permanently ban you from receiving future paid services. (The same is true in the UK.)
Thanks for the warning. I am not on Medicare yet, but I imagine I will be when I get to the age they force me on it.
