Yes but that wasn’t a design fault in the machine. You drive off a cliff, you have to expect unwanted results.

It was definitely there in 1977. I was part of the casual labor (read “college students on Summer break”) who moved ~47,000 bars in support of a Treasury / GAO audit. And the other sealed vaults had plenty of gold, you could see though the ventilation holes.

Dean Kamen is very much alive. You're thinking of Jimi Heselden, OBE, who went off the cliff in trying to be courteous and give more clearance to a dog walker. Mr. Heselden had bought Segway, Inc., but had nothing to do with the invention of that device; he had made his fortune via the development of the "HESCO bastion" gabions used to fight flooding (from Katrina, for example).
