Isn't that basically the same thing, tell her to just keep talking VS. telling her what to say?

They're still telling her something. She's not acting on her own.

Well I think the difference probably is that the former shows that she is medically unfit and the latter shows that he is morally unfit.

I leave it to others to judge which is worse.
