I worked in an IT department for a large medical system. The first thing they asked at every encounter was do you have a healthcare directive and where is it stored. If the patient didn’t have one the provider was supposed to talk to them about getting one.

You bring up a good point: we’ve started telling our clients to keep their directives somewhere besides their medical records. Some are concerned it gives the docs permission to DNR you.
