Wiki says nothing about fatalities in its terse coverage but IIRC about 200 ground crew died. Estimated yield was roughly comparable to Texas City blast. Some debris found up to 6 miles away. Their manned moon program never recovered.
America has sent more citizens into space than any other; so yes, we’ve had more accidents.
