There are nursing homes in inner cities with half of those beds and the same death rate.

What type of skilled nursing home was the one in question?
Nursing homes have different skill levels or might specialize in certain type care of diseases.

Much important information is missing.

For example, if the Nursing Facility handles more renal disease and cardiac patients, the death rate is going to be higher than a primarily dementia facility.

But the article leaves that out.

10% bed turn-over in two weeks? 100% turn-over in five months? 200% turn-over in ten months? Just my opinion, but that seems excessive even for hospice care, let alone nursing home care. I'll make the bet now that this is excessive for the facility. Just a hunch.
