I realize this article is about care facilities in the UK, but we are seeing the same thing here: majority of deaths in facilities for the aged.

My wife used to work in a long-term care facility as a nurse.

Long-term care in this nation is a 3/4 of a Trillion dollar industry. Yet, she found that most of the management was terrible. Instead of managing, they barricaded themselves in their offices and avoided other employees and the residents at all costs. Most of the orderlies didn't do their jobs, and that left even more burdens on nurses who might have had to manage 20-30 patients on their floor, including dispensing medication. The patients suffered, and the nurses did their best to cope.

As I get on in years, I hope and pray I am able to avoid long-term care facilities. I'd rather get older and die at home.
