The real failure was on the part of the police and prosecutors. Police were called to the house MANY times over incidents of violence and threats. If they had prosecuted him for at least one, then his name would have gone on the list.

And if you are posting videos of yourself cutting yourself & issuing death threats - then you’ve got a crime as well as disturbing evidence of mental illness.
