I also think it’s a cough drop. A hole in the tongue,that size ,would be pretty painful.
Perhaps MOH’s cancer surgery on a lesion that can be seen on the upper cheek of the previous smiling picture.
The surgery traces the root of the cancer for as long as it goes. Would have to be recent because the scar usually goes away.
