I am pretty sure I saw Trump shake the moderator’s hand.. Maybe a bit delayed, but he did it and with a strong shake and a hearty voice.
You are correct. Trump may have just wanted until Hillary had moved on before approaching Holt.

You are correct. Hillary made it to Holt first; while they were exchanging pleasantries, Trump gave attention to someone in the audience (fwiw, I too thought Trump was going to ignore Holt). Once Trump’s exchange with that person was finished, he went over to shake Holt’s hand.
