I saw this pic on another post and I thought it was 6th cranial nerve palsy. The 6th cranial nerve controls the lateral rectus muscle which pulls the eye laterally that is towards the side of the face. When that nerve is damaged, the eye ball will track inwards. A lesion could occur within the nerve itself, due to a localized loss of blood supply to the nerve or from a structure compressing the nerve.

Other processes that can damage the sixth nerve include strokes (infarctions), demyelination, infections (e.g. meningitis), cavernous sinus diseases and various neuropathies. Iatrogenic injury is also known to occur, with the abducens nerve being the most commonly injured cranial nerve in halo orthosis placement.[1] The resultant palsy is identified through loss of lateral gaze after application of the orthosis.”

All seem quite plausible, but there may indeed be a more sinister explanation, as null and void incisively and humorously speculates:
