There is clearly something wrong with her. This black guy has got her back on track by just talking to her and apparently by some accounts with a flashlight. But whether that object is an injection device or a flashlight this guy has it ready at all times.
While I would never wish ill on anybody but my first wife, I think she is getting worse and will not be able to hide it too much longer.

Watch carefully, not long after he appears, the light appears to flicker on the wall and door, so what he’s carrying might be a slim torch.
As you say, they won’t be able to hide it much longer.
