There is only a little more than 12 miles of I-81 in Maryland. IIRC, the big issue is the I-81/I-70 interchange.

I agree. I-70 intersection onto the already narrowed I-81 is part of the problem. Short on and off ramps at all the local exits don’t help anything either! It’s poorly designed.

I'm a Marylander and am familiar with the stretch of road he's talking about. The I-70 interchange is indeed the biggest issue. Otherwise, the author seems rather melodramatic - I've certainly driven on much trickier and more dangerous stretches of Interstate.
I personally think the I-81 stretch through central VA is worse than the Maryland part.
