I have had to find a new primary doctor as mine is to sick to return to his practice.

I will not fill out personal questions that do NOT pertain to my health...i.e GUNS, or personal info that is NOT related to my health. 0'care DATA MINING! sucks.

My reply to my doc was: Under advise of my lawyer, “I claim the right offered to me under the fifth amendment to the US Constitution to refuse to respectfully answer that question”.

There’s always YELP where you can provide feedback on doctors who cross the limits of professional conduct. Yelp reviews DO affect their practice and they DO care about it.

Remind your practitioner that you WILL post a review if you don’t like her conduct.
