This is because poorer Americans spend a greater share of their disposable income on tobacco, alcohol, and high-fat food than richer Americans.
I'm not a statistician, but wouldn't this be expected since one would assume that the ‘rich’ have more disposable income to start with and their bodies need the same number of calories? Wouldn't dollars spent per person make more sense?

This has been a pet peeve of mine for years. I’ve seen far too many “studies” (conducted by physicians with no research training) that show a statistical correlation between factors x and y, and automatically assume x causes y (or vice versa). No. Correlation does not equal causation. Causation actually has to be established by carefully designed experiments, and none of the “studies” showing correlations between consumption of *any* food and obesity have ever been followed up with confirmatory statements.
Obesity is caused by consumption of more calories than the body needs. That fact is both incredibly simple and almost intractably complex.
