GEnder confusion is one variant of a well-known mental illness called dymorphia. In other variants, people decide their right arm or whatever isn’t “really” their’s and needs to be removed.
If a young man decides his arm needs to come off, he’s treated for mental illness. (Not that our treatments are necessarily all that effective.)
If he decides his penis and testicles need to be removed, huge numbers of people celebrate his delusion and try to prevent anyone from suggesting he reconsider.
In a number of states, “conversion therapy” is now illegal, even for adults who voluntarily request it.

Could someone explain to me why men who identify as homosexual (attracted to the same sex) get into relationships with men who dress as women or even who have their sexual anatomy on top and bottom altered to resemble the opposite sex (and take female hormones to become even more feminine)?
Seems a lot of work for someone who doesn’t even like women.
