Maybe the form is a forgery. I remember the 2000 election where it was reported that the Miami Dade county election commission was 100% Democrat and was responsible for the butterfly ballot that allegedly confused so many voters.

In a 2009 voter-registration application, obtained from the Miami-Dade County Elections Dept, Jeb Bush marked himself as "Hispanic" in the field labeled race/ethnicity. Mmmmm....I just can't wait to see what that revelation does to Jeb's poll numbers.

The butterfly ballot confusion occurred in Palm Beach County, where sugarcane cutters from the Caribbean voted by the busload.
Miami-Dade County is home to 15 Peruvian restaurants and 20 Chilean restaurants. Where do they get their clientele in a country of 133 million Americans?
