How do they know it was a pilot knocking on the door? Do they have any idea what was going on in the passenger cabin?

They don’t know. Could be both pilots were poisoned. Pilot one became ill and left hurriedly before a crew member could get in. Pilot two fearing terror blocked the lock then also succumbed.
Air crew tried but couldn’t get in.
Doesn’t explain the descent though.
