The Obama administration on Monday announced an ambitious goal to overhaul the way doctors are paid, tying their fees more closely to the quality of care rather than the quantity.
Rather than pay more money to Medicare doctors simply for every procedure they perform, the government will also evaluate whether patients are healthier, among other measures. The goal is for half of all Medicare payments to be handled this way by 2018.
Monday’s announcement marks the administration’s biggest effort yet to shape how doctors are compensated across the health-care system. As the country's largest payer of health-care services, Medicare influences medical care generally, meaning the changes being initiated by the administration will likely be felt in doctor's offices and hospitals across the country.
“As a very large payer in the system, we believe we have a responsibility to lead," said Health and Human Services Secretary Sylvia Mathews Burwell in a press conference. "For the first time, we’re going to set clear goals and establish a clear timeline for moving from volume to value in the Medicare system.”

Doctors are already retiring rather than dealing with the mess that the practice of medicine has become. I was in pharmaceuticals and got to know a lot of doctors. They (almost)all are disgusted, tired of being second guessed by some idiot with a computer screen telling them what is best for their patient. My primary care physician retired a year ago at 60. He was a great doctor, a nice guy and just didn’t want the hassle. He still teaches, fills in at a clinic occasionally.

Don’t forget that many here on FR did not vote at all. By not voting, they put Barry in office.
The logical outcome of this policy is that if a doctor believes that an incoming patient might reduce his/her cure rate, the patient will not be treated at all. This accomplishes the goal of some in government who want to stop all healthcare for those of us over 40.

I just can’t wait to experience “quality” care over quantity care. Even getting a pain pill instead of a hip replacement will be questioned all the way up the insurance chain
I see this euphemistic little announcement as the start of an accelerated silent genocide against the elderly thru denial of care, all in the “best interest” of the State

Oh, I think they might. Rather than any costly procedures, docs will give advice such as "eat more healthy things, get some fresh air and exercise, and take some aspirin for any pain."
The "ACA" experience won't care how many patients "die off," as long as they do so in a timely manner.
For years, doctors have had lawyers breathing down their necks, now King Obola want’s government functionaries to take over that task.

It might be comforting to think that, but it's simply not true.
Let me help. I've been involved in "quality measurement", or more accurately, failed attempts to measure quality, for 20+ years.
If you're a doc, you know quality when you see it, and you know the opposite. But the folks are not like that. They don't know, and, because the highest quality still brings you (news of) cancer, stroke, impotence, decline, and death they don't WANT to know.
Every single nationally imposed quality measure has failed, and failed dismally. That's the bad news.
The "good" news is that the quality industry, the payers, and the government are finally agreed on what to measure and how to measure it.
In the future, everything - payments, staff privileges, discipline, licensure - is going to be driven by "patient satisfaction".
I'm not kidding.
Well, 90% patient satisfaction and 10% following guidelines.
That's it.
In 2024, Dr. Oz will be the highest quality doctor in the land.
My doc was part of a 5-doctor practice. They allowed themselves to be bought up by a corporation which runs several hospitals in the area. Now they are just employees of the practice.
This is the long-term plan: to eliminate small private practices and have everything belong to a few corporations, which will be easier to control, and who will pay large campaign contributions to their political masters (if they know whats good for them)
So the only way to be well paid is to take healthy patients.
No risk medicine.
It actually isn’t only the elderly. I am hearing about terrible health care in a number of different situations emergency room situations.

There have been quite a few on here boasting that they did not vote. I read their posts myself.
Yes, of course there were some FReepers who didn’t vote.
But they didn’t “put Barry in office”, as you asserted, unless you meant that the general voting population, including some FReepers, was so turned off by the process and the candidates that they chose not to vote for their own reasons, or they just didn’t have the motivation to vote.
Unlike some countries, Americans are not required to vote.
If anyone chooses not to, that’s their business, and I see no reason to criticize anyone for that decision, especially for the reasons that these non-voting FReepers stated.
