I hate the whole idea of some appointed Poobah who is nothing more than a figurehead and mouthpiece for the administration.

Obama had a candidate for Surgeon General. The person is a far left know nothing hack with no experience. That person could not make it through the Senate. That is why their is no acting Surgeon general. It would make no difference you see.
