Not necessarily. Plenty of men on the “down low” then proceed to have vaginal sex with women, thus spreading HIV to women via heterosexual sex.
Something like 80% of new HIV infections are homosexual or needle-using drug addicts. Given they are such a small percentage of the total population, even after 30+ years of HIV’s widespread introduction into North America, its still mostly a gay disease.
