in looking at the picture the area around the lungs looks like he had a blow out, or dragged and the body became dislodged when something broke on the vehicle. Cause of death - speculate - ruptured spleen, collapsed lungs, intestinal hemorrhage, and he died of a broken heart.

The ole in his chest was an emergency thoracotomy to attempt to clamp his descending aorta and start his heart up. It would seem it didn’t work (rarely does).
