Indirect fire is basically fire coming on your position from a weapon you cannot see or return fire on. Basically a mortar, howitzer. Direct fire is from an AK-47, RPG, etc..., that you can see and engage through your personal small arms.
So she died from an explosion, shrapnel from a mortar round probably.
