The steam tubes were failing/near failure. I would hazard either the metal used was faulty or the operation was beyond normal parameters. 22 months is way too short of a time for normal usage for a normal tube.

Pretty much, they used steel from China and SURPRISE! it wasn’t made to spec, seems the Chinese cut some corners to squeeze out a little more profit.
