When my son was born in the Portsmouth Va Navy hospital in 1999 all we got from the state of Virginia was a computer generated COLB. When I asked the hospital about it, they said that everything is electronic now - there are never any paper copies. All the information is collected electronically and sent electronically to the state.
We had no problems getting him a passport.

I had to provide an original long form BC with the raised state seal on it before I got mine.

Or, could you tell us what items are on the COLB?
Do you have a long form birth certificate with the doctor's name and the name of the hospital on it?
If you asked Virginia for a long form birth certificate, would Virginia give you one? Thanks.
